You will gather data about your heart disease and brainstorm the causes and social determinants that impact this health condition. Part 1 is graded individually. You will be asked to find the following data and submit it.. Section 1: Data about your health topic (5 points) 
Your name and your health topic (heart disease) For your health topic(heart disease), find out the most recent U.S. statistics on: Incidence & prevalence # of deaths & death rate race and/or ethnicity Gender (if applicable) Provide a link to your data source for each stat or you will not receive credit

 Section 2: Is this problem getting better or worse? (5 points)
 Use at least three points in time to make your assertion (for example, provide incidence rates for the years 1990, 2000 and 2010). Answer in 3 paragraphs or less.

 Section 3: Social Determinants (up to 40 points)
 Identify up to 10 Social Determinants of Health related to Heart disease. Be creative. (up to 20 points) For each of the Social Determinants of Health you identified, write 1-3 bullet points relating it to the health issue you chose. Think about how these determinants impact your health issue and how it connects to the concepts we've covered this semester. (up to 20 points) The paper must include statistics and graphs and it must include links for every statistics and graphs used. the paper must be at least 850 words in length and include at least 6 references. the paper must be in perfect English.
