Document Form Creation

Hi. I would like the Annual Talent Assessment form to "look and feel" like the Vehicle Inspection form. Also, the Annual Talent Assessment form should be limited to as few pages as possible. The form needs to be in a fillable PDF (minus the signature lines). Perhaps instead of the sections being horizontal, they can be vertical to free up space?

Finally, a "Goals Section" needs to be added. Four areas need to be included and have the ability to type in. The areas: Start, Stop, Continue, and Resources Needed. There also needs to be a "key." Start: Will create value; Stop: Does not create value; Continue: Creates value
