Form Builder - design a form (template) using Word or XL (other software considered)

In my job, I interview and coach people. I need a form designed (template) which I can use during coaching and interviewing. This is not an online form. It will sit on my desktop and I need to be able to share it with others. I use a certain coaching procedure and I want this form (template) to follow this procedure. It will be divided into sections, have rows and columns, make use of keyboard shortcuts and have some colours or shading. I don't want it to look like a spreadsheet. It should have white space with most of the rows and columns hidden. It should be aesthetically pleasing. It may have shading or colour to high-light certain sections. There will be a several fields that have names. The rest of the fields will be free text fields that I can type into. See attachment. It's not complicated but has a couple of small challenges which I will describe to the successful freelancer. I need someone with advanced form building skills.
