need designs for several web apps

I have several web apps that are already built but I would like to redesign them to look a lot better, they need to look great! I would like to begin with a 1 page test project just to make sure we both are happy with the work, and the requirements are met (by both sides). What I need from all designs is very simple: 1. Very well formatted PSD files of the design. 2. Each app should have its own main theme design, so all pages under the same app should follow the same design pattern. 3. You will get a mock up (or already built design) of what i think it should look like, you will need to redesign it so it will look better. I would like to start with a test project of the attached mock-up. This mock-up is of 1 page of our app that shows the settings the user can change and save, as you can see this looks bad and as i said, a mock-up and not a real design. All the titles and inputs and the text in the mock-up must be included in the final design, but the placement of them can change, so you are free to do any design you would like, but the main color must remain the same (#276EF1), and the final design must include all the elements from the mock-up.
