Modify an HTML5 document to add Signature and Camera control (Re-post)

I have a HTML 5 document that is displayed on a current model iPad Air with Safari 7.0+ that needs to be modified with a couple of snippets of code to be to have three functions added: 1. The document needs a signature pad where a signature is needed on the last page. The pad should be a pop up window that accepts the signature and then displays it on the document. The signature needs to be responsive and have no lag. Third party libraries are acceptable to use. 2. Using HTML Media Capture the document will have a small camera image area that will be displayed on the client-side without uploading 3. When the SUBMIT is clicked the camera will take a picture of the person signing, the signature and picture will be saved as an image to a database and the picture will also be displayed on the screen in the lower left corner of the document. Code needs to be written in HTML5, JavaScript and jQuery I've uploaded the HTML 5 document.
