App to save incoming call and show caller is from my db

I need an iPhone/iPad app and android app.

I need a call wrapper sync with my database.

I want to save incoming call: caller, date time and duration.

And if is possible I want to intercept and show if the caller is on our database.

I can provide MySQL database and ftp Area for make Php side.

I need apk, ipa, source code and Php on the server
