Create a cross platform Mobile App

I currently have a App used by a travelling sales person and the sales person uses it as a point of sales application. Currently it is a Microsoft Access program running on a Windows tablet.
This project it a proof of concept. I want to recreate the program using a cross platform mobile app so that it can sit on a small tablet.
I do NOT want to create seperate native apps - I would prefer to use something like PhoneGap or another similar platform to create a cross platform app.
The database is already created and the program is already developed (in a different technology) so I do not need a team or designers or requirements engineers I need someone to start coding

the concept is that there are "routes" and a route has shops in a sequence and the driver selects a route and on that route there is a list of shops that they must visit that [login to view URL] all shops will purchase something but the driver must visit that list of shops in order
Once the "route" is selected the driver arrives at a shop and selects the "shop" on the screen. Once a shop is selected the driver is then presented with a list of "products". On this screen the driver is also presented with a sales average and this is a simple calculation of the average sales per week to that shop. In other words what that shop normally buys and sales history is abailable in the database.
The driver then scans the barcode of the product they are selling with the camera or manually selects a product from a list and the driver adds the quantity of that item then the next item and the quantity of that item and continues to scan product into the shop. Once all the product is scanned in they driver completes the order and it presents a order summary for the shop owner and if the owner is happy the driver commits the order to order table and prints a receipt / invoice and moves to the next shop on the list.
Again a demo / video can be provided to the winning bidder with the right technical background.
It is proposed the app would be created in PhoneGap or a similar platform using HTML and CSS. The app is connected to a cloud based web server with a mySQL database and we will call that the master db. the apps should also have a local db of some kind incase connectivity with the master db is lost and there must be some sort of method for syncronisition from the master to the local db when connectivity is restored. There are about for or 5 screens used in the work flow
I am looking to produce this as a proof of concept. I am hoping this will replace the existing system but i need to see it as a proof of concept and because it is not a finally polished app I can live with a version that will need future work to complete to a production level - this will also affect the price. I am not paying for a full polished production version I am looking a proof of concept so this bid must be lower to reflect this and if this does work I would return to the successfull bidder to continue development.
Finally I am looking bids from developers who have created apps using something like PhoneGap. I want to be able to see something in your portfolio that shows me you have doen this before. I would liek to see you have created an app that uses a local / master db so past performance / portfolio is very important
I am expecting that because this is a proof of concept and that all the design and database work is already done that the cost of this will be around $250 max (an that is a MAX not a target - if you are below that you stand a better chance of selection).
