OCR scan to extract directory entries from book or possible .pdf format

The requirement is to scan via OCR and store and export that entries found in the publication text. Each page is composed of 4 columns with entries containing name, practice name, address, telephone, fax, e-mail. The are composed in each line and they are all systemically, or the data print of each required entry is easily formatted. Each data entry is required to be exporting to a .csv, and the whole document should contain a set with each person / practices name as grouped element within the set. The format is a comma delimited for import into data system. There were not OCR programmes that meet the requirement, though scanning each entry would be good.
