Form creation

I am with 1&1.com. They support CGI, .asp, PHP, PERL and JAVA, but don't supply base programs for use with add ons. I want CGI, .asp, or PHP but don't really know one from the other.

I have a scanned form with table framework, boxes, signature lines, etc. I can convert this to an editable Adobe .pdf form or create a CGI script to get the information. However, that doesn't work as is.

I need to allow a non-experienced web based surfer to enter the required information in some online form and hit a submit button when they are done so that the final product emailed to multiple destination looks as if had been printed, typed in and then scanned before being emailed. The recipient must be able to receive it, select print, and have it print out as a completed full image form. IOW a final graphic file showing the original form filled in would work, but without the requirement to print, fill in, scan, and return the image by the surfer.

One of the recipients does NOT have the ability to transfer a data stream to a printed form and they do require the "approved" look of their form to accept it.
