Need to connect contact form to a MYSQL database

Need to connect my contact form to a MYSQL database I’ve created a contact form through www.phpform.info. It will send the information to me as an email, but it is also supposed to add the information to a MYSQL database. I logged into cpanel and created the MYSQL database, but I don’t know the php code to connect my database to the form so that it can write the incoming data into the MYSQL database. Tried to find the code on Google but no success so far. The form html file that generated through [login to view URL] is attached, as is the script that phpform also created. So I need a freelancer to add the code so that it connects to our MYSQL database. The online system created a spot for it and put this text into the PHP script: #add your code here now or later to connect to the MySQL database. I also want the following: 1. At the bottom of the form, add a simple security question to help block out spammers. It can be something easy, such as “Please type the answer to: 2 + 2 =” 2. I want an Admin page, where I can log in with a user ID and password and then look at the files that are in the MYSQL database. I need you to show me where I can go to change the Admin password from time to time. 3. I can click on a button on that same Admin page, and then download all the records as a .csv file to import into Excel or other software. 4. I want to add the function where the script will send a confirmation thank you email to the person that just filled out the form. The thank you letter will contain all of the information that they just provided to me when they filled out the form. The php Admin page needs to be dummy proof so that an inexperienced computer user can work with it easily. For someone who knows PHP and MYSQL, this is a quick and easy job.
