Affiliate website design/integration to mediaalpha, katch media, surehits, brokersweb, etc..

I am looking for someone that can design a website that collects a simple zipcode on it for an affiliate auto insurance website, and then integrates with either katch media, mediaalpha, or Surehits (quinstreet) code where their code onto my website. After the user submits their zipcode, I would have the offer directed to another url of my choice. I need the ability to have multiple landing pages whereby some of them direct to website A, another one to website B, etc.. I would also like to add the ability to have a back button redirect, and push notifications added. I will need the ability to change this langing page for specific media placements whereby some of the landing pages do not have the backbutton redirect in it, or the code for mediaalpha/surehits/brokersweb as well. I also want to be able to add a phone number to the page and be able to edit the phone number easily. If you have any experience integrating code onto websites that work with mediaalpha,katch/surehits/brokersweb, that would be a big plus. I will be adding many more websites like life insurance, health insurance, etc.. in the near future as well. For the backbutton redirect, I would like the best solution that can work with the most browsers, and be kept up to date since browsers are constantly changing and effecting the ability to use this script. I have scripts for collecting a zipcode and sending it to another page, and have found some for backbuttons as well, but would also like advice if there is something better. I need someone that knows how to optimize for each browser, and optimize for mobile as well.
