Build an iOS & Android app for me based on current database

Hello, I know I have placed a bit higher price but I am able to make this application either on Native or Hybrid platform whichever you would prefer. So basically the app will launch a form with some text boxes as inputs by the user and admin can manipulate those data from their access. Please let me know what else you require from user and admin perspective so that I can give you a realistic quote. Also I would like to make sure what you actually meant by working on desktop app?
