Conditional Web form output to CSV

I need to host a web page on IIS that will display a web form (ASP / PHP /Java up to you, Basic CSS HTML) with a set of fields (in attached file). When the form is submitted it should create a CSV file with a header and information provided in the form. One input field in the form is 'applications' which should be set of tick boxes i.e. app1, app2, app3, sql, IIS etc. For example if 'sql' application is ticked more input fields should be made available, related to SQL application. These fields are in the attached file. One input field is size. There should be a hyperlink under this field which should display a size chart on the same page when clicked. Size chart is a table. Windows username should be also captured in RequestorName field automatically. Details of all the fields are in the attached file with default and possible values (in a drop down).
