Create website clone from old CMS site - 12 pages

We are moving web hosts and not continuing with the current CMS.
I do not have access to the existing ftp, but the designer gave me a zip of all of the images.
The website should appear and function exactly like the existing website.
The resulting website pages should be HTML,HTM, or PHP files.
Navigation pulldown menus, Google Maps and Analytics, PHP contact form, should all work exactly like the existing site.
The resulting code should be cleaned of unnecessary external comments, references and links.
I will provide the website url to potential freelancers.
