Move custom module from one place to another + disable customer registration - MAGENTO 1.9

If you cannot do this project in the next 4-5 hours please do not bid on it, I need this done very quickly. I have a custom module in the MY DASHBOARD section of Magento 1.9 site. I need some of it moved to a checkout step. The module allows a user to add some custom content to an order, but my client wants the module at checkout, not in the user dashboard. See screenshots, it's pretty self-explanatory. This module will replace SHIPPING step on checkout page. This module does A LOT of other things that post back to the database as well as data gets pulled into the admin on order pages and also on packing slips and order exports, so none of the existing functionality can be lost. The form just needs simplified and moved. Also need to allow Guest check-out only, no registration or sign in by a user to check out. SO when user clicks to check out they go right to the form, they do not see the registration or login form. Again, this needs to be done in the next 4-5 hours. It is 4:20pm EST right now, I need this done ASAP. If you cannot do this now please do not bid. Please also ask any questions now, I need to find someone ASAP to do this right now.
