Online quote

I urgently need to do the following work:

I have already developed a web interface, where I need to insert an online quote.

What do I mean by online quote?

That I can filter based on three things:

First in terms of specialty (they are 4)

Second (depending on the selected specialty), select a sub specialty.

Third select the urgency: there are three alternatives.

This has to be left with an example (css) of the product which I myself can load the products and condition them sensibly.

I have basic knowledge of web programming.

It is something very simple that will not take more than 2 hours of work.
