PHP App - Bill of Materials for Furniture Maker

See attached scope document. I have a Microsoft Access App that is currently used as a production and costing system for a furniture maker. I want the system to be replaced with a web based php/mysql system. Cogeigniter or Laravel. All source and DB to be handed over to me unencrypted after project completion. No chancers. Company's will be given preference over individuals. High ratings are beneficial. This project is time and cost sensitive. My budget is $850 USD MAX. Time frame for development is 4 weeks MAX. No deviation from this will occur. I need constant updates. I want a test version of this to be installed on my test server so that we can test. I will test and send you feedback to fix until the system is completed. No advance payment. Full payment will be released once the project has been successfully completed. Milestone payment is possible of course. No payment until completion. No excuses about you needing money for anything will be entertained. This is a professional endeavour. Thank you.
