Wordpress Expert Needed for Fitness Center Website

The following site: [login to view URL] needs some updates & fixes. Possible opportunity for ongoing work for service provider. The website has a form on the Home Page in the upper right portion of the page under "Sign Up For Your Free Executive Gym Pass" that is no longer showing up. The form data is supposed to be forwarded to our "Intouch" CRM. This is no longer functioning. We also have widgets that are supposed to display scheduling from our "mindbody" account that is no longer being displayed on the "Class Schedule" page. At the bottom of the "T3" page there is a button ("Book T3 Class") that is supposed to take the logged in visitor to the "mindbody" T3 scheduling which will allow them to schedule a class. There might be a couple of links that are not functioning that need to be fixed as well but my main concern is to get the form to display and process the visitor's information into our CRM along with the "mindbody" scheduling widgets to display on the pages where they are not currently being displayed. Theses features were once functioning properly and can be seen in the backend of the site but are no longer displaying on the front end. The old webmaster/developer is unreachable and I'm looking to get these features functioning again and for someone to possibly take care of updating the website moving forward.
