Write Code that will implement a subscription price calculator to my wordpress website

I need a code plug-in that I can use on many of the pages of this website which will help customer calculate their monthly subscription. It is not 100% straight forward like 1 + 2 + 3. But instead you have different subscription combinations with different prices. You would have main section one: ENTERTAINMENT with selection Sky Original or Sky Variety - that is the BASIS you select. Then you can add Sports, movies and HD. All of them or one or two. It changes the price every time. See image attached showing a rough sketch. Oh, and I want to be able to edit the pricing! So, there needs to be some sort of table allowing me to change prices myself.
