Online forms for database

We are a sales company with many contracts to sign with customers. Forms move in and out and back between our company and client. Our plan is to do the deal in two contacts with the client. Confirmation from the ID of the phone is required. The software uses ID number to interogate credit beareu database to verify and download clients information on debts. the agent must be able to print through all forms of printing from smartphone or any other device. cordless printing is prefered. The system must be cloud based. The app must read out from a recording relevant to where the client is filling to explain the point that the client is signing on. The client must also be recorded in answering question. the answer would be yes or agree. The client must also sign on the phone with relavant equipment to be carried by agent. The client must also be able to fill in the form based from information obtained from the website without the agent. Client must be able to upload information by photo, fax, scan or email.
