Help With spss

Briefly just what I do and need help with. I have to make a compilation of a computer matrix where about 60 people have answered 40 questions. (20 questions before diagnosis and 20 same claims after diagnosis) comparing treatment from the environment before and after diagnosis. The answer options have been 4. In grad scale from agreement to not agree at all. These responses indicate positive or negative response from the environment in four degrees.
Now, with these data, I need to t-test when I compare tm-eating before and after diagnosis. I need help with SPSS to make a division of the questions (I have the groups ready) so that they are grouped in about 4 groups each group needs t-test before and after diagnosis. I wonder what's more needed like average and other televants that must be included in a performance assessment.
