Singleselect/ Multiselect form

I need to create a multiselect form where I can also make it as single select if I want.
I need to have an option on the form to be able to:
1. Searchable: true false
2. Multiselect: True false(if false then its single select)
3. Countable: Lets say there are 10 multiselects selected then, it will say 1st name + 9 . SO, basically counts how many selected but will show the name of first item selected
4. Selectall/ Unselect all option: Useful in multiselect when I might have many option.
5. Lastly, the selected option should be in URL parameters when multiselect is selected. For example: Lets say there is multiselect for Foods and has value such as apple,banana,orange. And If apple and banana is selected, the in the url it will be: [login to view URL],banana.
More example: If there are multiple multselect in same page, lets say we have already ?food=apple,banana active in the page and if we select drinks=coke,fanta,dew then url would be
[login to view URL],banana&drinks=coke,fanta
When url goes directly to /?food=apple,banana&drinks=coke,fanta,dew it should show
[Apple + 1] in food selected and [Coke +2] in drinks selected.
No library should be used unless it is less than 200~300 lines on code.
