Have a small Microsoft Access 2010 (MSaccess) application that need to convert into a basic web application. The resulting database should be mysql and the forms/reports should be in php. Also there should be a user management module created. The MSaccess app consists of approx. 4 tables, 15 queries, 15 forms.

Looking for a quality quick turn around on functionality (not design needed)
