Build a sign-in page using servlets

I need to have a page for users to authenticate using servlets (using java annotation)
The page should look like this: fields:username,password and "i forgot" link
When User is not exist, or bad password , a string should be populated to user (on the same page) with error message like "Authentication Failed, Wrong username/password"
The container is Tomcat.
users and roles should be in mySQL table with fields users(username,password) roles(role_name=users)
The page have to have forgot password function with email reset password option
This Should be nicely integrated into existing java web-app in eclipse.
