Update our website application form to PHP7

Hi there, we have a PHP application form on our website at: [login to view URL]
It is old and works on PHP5 ok, but doesn't submit when we upgrade to PHP7.
We need someone with PHP experience to troubleshoot / update the code so it works perfectly in PHP7 (with all it's current functionality: eg. auto rejection criteria, auto email sent to applicants after submission etc).
