Survey script needs two online survey pages.\n\nRequirement:\n1. Need to replace two online survey pages with only one new page containing two fields (html page supplied and attached).\n2. Load thanks page (html supplied and attached)\n3. System sends email if user input < 3. Change email send criteria: send email if first radio option selected.
