Data entry form that automatically populates new business register

I am looking for a freelancer to develop a data entry form that can automatically populate my business register. The form needs to include various pieces of data such as contact information and financial information. I would like the form to be built on Microsoft Excel, and for it to auto-populate the business register according to requirements that I specify. This is an urgent project that requires a high level of accuracy and attention to detail. I would like the business register to populate once the advisor handoff sheet is complete. The advisor handoff sheet has to be accessible via sharepoint. it must be saved automatically to the folder with client name & 'advisor handoff' and date 'yymmdd' as name of file when complete -- so we can manually move to different folder. But the template that was used resaved as the blank document to be ready for next user to fill in. The new business register should be populated with latest form information being insterted in the top line of the records. No all records arer just one line .. see attachment. Attached is advisor handoff and new business register. The info from the advisor handover sheet that needs to populate the business register is shown in the green columns A-K on the spreadsheet and is from the first section only of the advisor handover form. The rest of the advisor handover form is not needed to be inserted into the business register.
