Create Print and Packaging Designs

Hello, I would like someone to re-create a Dr's Script and pathology request form for us to make it look a lot more professional. We would like company logo added. Possible some borders, we would like the script to be all black and white. Black and white logo attached.

The pathology to request form can have a colour logo.

We would like this to look as professional as possible this will need to be completed in Microsoft word. Everything that is on the forms now needs to be on the re-created forms. We will need to be edit name date etc. Happy to pay AUD $20 - $30 for this.
