Need a wordrepss Booking Form that looks up pricing from a table

I am currently developing a website for a taxi company that need an online booking system to allow customers to make bookings.
At present they have an HTML site that has a booking form online. This form is completed on the front end and the pricing is retrieved from a set table on the backend of the site. However having looked for a Wordpress plugin to achieve the same there is some out there but nothing that can do it all.
The form needs to be able to retrieve pricing based on car type, to and from points and then time of day booking. Also there needs to be another option to display hourly bookings for a tour option.
The existing website can be viewed at [login to view URL] where you can understand better the requirements of the job. I have also attached some screenshots of the backend to show you how this is currently laid out and again get a better understanding of what is required.
Either a solution using an existing plugin that you can implement and complete the work on including the design of the actual forms on the new site or a complete custom plugin to achieve this result.
