Rebuild a ColdFusion/SQL form with calculations in PHP/MySQL

We have a multi-part form built in ColdFusion:
page 1, enter information
page 2, upload files
page 3, receive calculation totals
Example of calculation:
<cfset tFinalRate = (premarr[7][#arcol#]-premarr[8][#arcol#])+premarr[9][#arcol#]+premarr[10][#arcol#]+premarr[11][#arcol#]>
The final result will be emailed to a specific email address in order to provide the user a quote.
Notes:
We will provide .cfm files and SQL database.
We will expect an NDA to be exectued.
