Add a Form To a Webpage (hosted on Free Hosting plan)

Add a form to the following webpage: [login to view URL] Please use the exact layout that I already have on the webpage. The page should have fields so the user can change the quantity of each of the 6 items listed for sale. Add a field so the buyer can choose one of the shipping methods listed. The form should then automatically calculate the total cost of the items ordered including the shipping cost and display it on the webpage. The form should also have fields for entering the buyer's shipping and billing info (as indicated on the webpage). And lastly it should have fields for the buyer to enter their credit card details. Then please add a button to send the order details by email. All the information from the fields should be sent by email: Quantity of each item ordered, the shipping method selected, total amount, shipping and billing info, credit card info. Please note that I have my website hosted with a free hosting plan. If you can not work with free hosting please do not accept this project.
