Create order form

Hello dear people, For our bike shop we need a job order which we can fill out if someone orders a bike from us. This block should then be printed as a durable block. The block should contain: our logos, contact details. To fill in should then be: name of the customer and contact details, fields filled in to the front where you put the price in the last column, a total at the bottom, etc. I have already written everything in a Word document, now it has to be put into shape. If anyone is interested in it and can do it well please contact me. Thank you Hallo liebe Leute, wir brauchen für unser Radgeschäft einen Auftragsblock den wir ausfüllen können wenn jemand ein Fahrrad bei uns bestellt. Dieser Block sollte dann als Durschlagsblock gedruckt werden. Der block sollte enthalten : unsere Logos, Kontakdaten. Zum Ausfüllen sollte dann sein: Name des Kunden und Kontaktdaten, vorausgefüllte Felder die zum ankreuzen sind wo man in die letzte Spalte den Preis einfügt, eine Summe ganz unten usw. Ich habe bereits alles in einem Word Dokument zusammengeschrieben jetzt muss es noch in Form gebracht werden. Wenn jemand daran Interesse hat und das auch gut kann bitte bei mir melden. Danke
