Single-page app for creating entries in database

We have paper sheets with the following fields filled in by hand, ten rows per paper page: First Name, Last Name, E-mail, Phone, Zip, (Y/N), (Y/N) I need a single-page HTML app that presents ten rows for entry of these columns. The user will enter the data and then hit submit. The program will validate the e-mail and format the ten-digit phone number to E.164 format, then create a record in a MySQL/MariaDB database running alongside the web server. If all records validate and are successfully entered, return a message indicating success and present another page of ten rows for entry. If any of the records do not validate, return those records to the user and give the user an opportunity to correct those rows. Once those rows are successfully submitted, return a page of ten new rows for entry of a new sheet. I would prefer this be written in Python 3, using either Flask or Django. Thank you!
