Converation of forms

We currently use a excel file which we fill out when we were calling the hotel and fill in the blanks and record the call and upload the call which is mp3 and send the entire message via e-mail. We have converted to web based but we are not familiar with the formulas in excel can be transferred to web based application. I need someone who can look at excel sheet calculation formula and where auto percentage can be implemented by using java/ jquery script for live forms or you can use excel for generating nice reports , pie charts or graphs. I need someone who is good in excel and can write csv file mapping filter & macros, Please find an example of the score sheet we use and we a total of 20 different score sheets. Please advise what the cost factor would be to do this. Once you have given me an idea of how long it will take and total cost, I can give access to the web-based program
