Develop a PHP website

I want web hosting order forms in php. I have 2 hosting products (Plans) in WHM Linux Reseller Account and I need two separate order forms for each plan. Process is Order Form fills --- > Instamojo Payment Gateway with Amount --> Create Cpanel Account if Payment Success and send server details by email to customer. Fields required in order form. First Name, Last Name, Email Id, Mobile No. 10 digit minimum and maximum (verify with OTP), Address fields country, city state, domain name (check availability from WHM exists or not), cpanel username (Auto generate domain name without extension, Hidden field), cpanel Password (Strong meter), Captcha Security, Terms & Conditions checkbox , (Hidden fields : capture system IP, OS, Browser, Location LAT LONG)buy button ---> after this instamojo payment gateway. --> if Payment Success create cpanel account and send server details by email to customer and show success page and store all these details in database with instamojo payment transactions details. Above form required with Bootstrap and AJAX and purely built in core PHP and MySQLi I will provide SMS and Payment gateway API's.
