PHP/SQL work for simple checkout and receipt delivery

This is probably about 3-5 hours of work for anyone with PHP/SQL experience. I hope to have this completed in 1 day. Don't bid if you don't have time to do it quickly, please. The mockup attached shows the existing design. There is little to no html/css work for this project, aside from making sure you don't break the layout. I have an older site checkout form with ALL of the existing code for the checkout and receipt delivery. This project will take the old code and move it to my newly designed pages. I will provide all necessary files; for both the old site and the new site that you will implement. On the purchase page, you will retrieve a list of prices from my database and populate a dropdown list On the purchase page, there is a "proceed to checkout" button that directs the visitor to Clickbank's hosted payment page. If the person pays successfully, Clickbank refers the customer back to a "success" page to deliver the receipt to the customer. This page has some logic built in that determines that the customer actually purchased, based on the response in the Clickbank URL. Again, this code is already written and will simply need some minor changes. All of the existing database structure is already online and available and I can provide all of the necessary schema.
