Website Design & Development - .NET ONLY

I need a developer who is GREAT and design and development. All projects will be web related mostly small websites. Technologies will include ASP.NET, C#.NET and SQL Server. If you do not have these skills DO NOT BID. For this project, I will post it here on RentACoder and the winning bidder will be the chosen bidder for all future projects through a private bid request or something. The First site is a dentist office. It will consist of 5 static pages including the front page and 2 dynamic pages. The first dynamic page will be a contact form that will simply email a specified email address the contents of the contact form. The second dynamic page will be an appointment request form that will ask a few questions and again submit an email to a specified email address the contents of the form. You will need to design the website and develop all pages. Requirements: ASP.NET and C#.NET SQL Server Excellent English Skills IM and/or Skype (US Number) ATTENTION: ALL BIDDERS MUST INCLUDE A LINK TO YOUR SAMPE WORK IN ASP.NET. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
