computer service clientele

hi i have a small computer service shop and i want to write in forma the name and second name of my customer adress and the number , mobile phone the type of the computer (laptop desktop tablet mobile phone or what else like a pcb for repair it) serial number of the machine one small box to tick if he give me a power supply or no. a place with 10 words to can write what is the problem of the machine . and at last a box with 20 words for me to record of service ( im whait for the chip 9800gt to replace and i have make a phone to my curtomer to know about service of the laptop) to i remember what i ve done to this machine because some times after 7 days i dont remeber very well what is the prob of the machine to end the service . i want to have the date and hour on it (it takes from computer). and one number different number for any customer. and with a drop down list if the service of the macine is at work or started or ended an it gone out of my lab. and at last i want to have the list of my customers in style like excel to find name or the date of the service and the phones to find it on the history. my english ar not so good but i thing to understant what i want. i dont a have a problem if the program is on excel acces sql vb or everything else
