Windows Webpage to use Authorize.net to accept a charge

UPDATE: The current website is in C#, not ASP.NET. I need a simple webpage, which can run on a Windows web server, to use the appropriate [login to view URL] SDK (C# one) to accept necessary information from the client, call [login to view URL]'s stuff to have them do a hosted page to accept the payment information, and then send an email to the client with some of the entered information. Specifically, we need to capture the client's name, phone #, email address, the Tour Name, the Departure Date, and Single or Double Occupancy, and the amount of the payment. The webpage needs to call the appropriate [login to view URL] calls so that [login to view URL] displays a webpage to collect the necessary payment information (credit card #, etc) and processes the charge. Then, this page needs to send an email with the above information to an address we will specify. The Freelancer will need experience using [login to view URL] on a Windows Webpage. It should be a pretty easy task, and minimal design will be needed. I may provide a logo and page title, and I'd like the page to look good, but nothing special regarding graphics are required. I'll supply test [login to view URL] credentials and test credit card numbers. I don't currently have access to a windows web server to offer for testing. I will need installation instructions of how to install/setup the resulting files so that they can be used, and where to modify the [login to view URL] credentials for when we move from test to production.
