Subdomain based application design sample

We are looking for a sample project design that will include a one registration form and at the time of registration we ask customer to chose the prefered name. that prefered name will become the subdomain for the user and it will created on fly. once the subdomain is asigned then the user can login with there user name and password by using the sub domain url and all the file, folders and database need to get copied to that subdomain of customer over server. the server will be IIS 8.0 and database will be SQL 2012, we need the sample coding for the above mention stuff along with configuration support for server. the task is not required any design layer and we will provide registration form, sql database and other project informations. we need the task to be completed in 3-4 days
