Survey Creation Software

I want to create a SaaS website with Kendo UI and sql server database, where I can create dynamic forms/surveys similar to "surveymonkey". The key features will be 1. ability to create the survey with multiple questions and allowing responses to be text/ date/ dropdown/ checkbox/ Radio button and a few more standard controls. We will also allow user to be able to upload a file (xls/ pdf) at end of the survey. 2. Distribute survey link and allowing people to fill it - anonymously or as a registered user. The site has to be responsive. 3. User registration 4. Admin panel with ability to view survey responses and download attachments Please provide serious quotes only. It is required to use Kendo UI in front end and sql server database in backend. Before awarding 1. I will like to know similar work done before 2. Code sample / reference site 3. Your approach (proposed milestones and delivery cycle) Development will only start after prototype has been approved.
