ASP.NET mvc project survey.

My project is a simple survey in ASP.NET MVC, I was building my project from a template sadly I am kind of stuck. I basically need to add a checkbox generator to my survey apart from the two options that I already have. My application allows the admin to create a survey with two types of questions and I would like to add a third question type which is a checkbox type. In other words the admin should be able to input the options for the checkbox and see the results in the Results View. Please have a look at my application in order to understand the premises.
