Hi there, I have a website survey page that has been more or less done, but requires some additional fixing on it. URL : [login to view URL] - I need this responsive template to allow all fields to work for MOBILE and TABLETS as well. (Currently the tables break when viewed on mobile, not usable) - Ensure that all fields are required fields - On completion, to send email with fields to admin email - Save all fields to database, I need an admin page to export entries too in viewable excel format. Budget : $30 (another $20) for the backend admin page and export function. Deadline : Today or latest tomorrow morning Thanks.

i have gone through the url you provided, and observed this issues 1) NONE of the feilds in screen 1 are validated (except email Id) i.e if user is given text in place of age error is not shown 2) required message is not placed in correct position 3)jquery validate library is throwing some random errors 4)some to markup has to be made to make it responsive(using bootstrap already included) i am new to this website , give me chance
