Create iOS Form Based On Existing Web Form

We are looking for someone to help develop a form for an iOS application. - The form will be based on an existing web form which has 16 questions including date field, location (lat/long), single and multi-selection choices, sub-selections based on initial selection, email address, etc. - Some fields are mandatory and some require validation. - Some fields should dynamically show or hide based on previous input. - The form needs to support iOS 7 and iOS 8 and needs to work well on iPhone and iPad in both landscape and portrait. - The form needs a header with Cancel and Submit options. - The form needs a database field name associated with every option to enable the data to be collected correctly. - The form needs to support programmatically setting values based on previous submissions of the form. - The form needs to be easy to use and attractive. The initial task will be to suggest a design for the iOS form, including any required third-party libraries. Implementation can commence once the design is agreed. On completion, all sources will need to be provided (with meaningful comments) and then you will need to assist our development team to incorporate the form into an application we have under development. The winning bidder will need to be available to talk over skype in Australian east coast business hours with good English skills. They will need to liaise with our development team to ensure the project is a success. A quick turn-around will required. Ideally the project would be completed before Wednesday the 18th of February.
