Smal MySql database job

I have a website that is running Joomla. Im creating a form with multiple fields. Each field has the ability to have Multiple values. I have a field that I need to add hundreds of values and I dont want to add them one at a time in joomla. Instead I can click a button and it says "Data stored in an external table". When I do it calls for: 1. Database table name 2. Column name for label 3. Column name for value I have a .rtf file with all of the names separated by commas that I need to be uploaded to a new table. So I need someone to take the file and create a table in mysql and then tell me how to upload the new table to my database.
