Interactive PDF Fields

I have a pair of pdf forms, which need to be made interactive. Specifically, the forms have multiple areas on each page that a user would be entering their name, insurance policy numbers, and financial numbers to complete the form from their computer. (basic text entry fields, some with formatting for financials $ xx,xx) The goal is for the users instead of having to print out the forms, and then enter the information manually with a pen and then scan or fax it back, to be able to open the form in their browser window and then use the keyboard. The fields should be individually named with numbers, and in a logical order with field #1 starting at the upper left corner of the form and progressing in a left to right fashion, ending with the last field being in the lower right hand corner. Need all fonts to be Arial 8pt
