simple php mysql ajax form

We need a simple set of forms in php to list, insert, edit and delete values from one mysql table. (9 columns) The forms will be used in the admin section of our site to manage banners in our categories. Ajax knowledge is required as the forms must use ajax for easily selecting in what category to insert the banner and what product does the banner refer to and these values will be selected from 2 different tables by typing the first letters of the category or products name (ajax live search) No site modifications are required, the 3 database tables will be provided. So we need: - one file for listing banners from an ajax selected category, with a delete and edit button for each value. - insert form for new values - edit form for existing values The sql file with the structure for the 3 tables is attached,
