Design Finance Website and PHP Coding

There are two main things I need done. I am wanting a website built to collect user information. I want the website design to be simple, but professional looking. I don't need a lot of unnecessary "junk" or excess stuff for my site. All I need is a "contact us" and "privacy policy" link and a little bit of UK financial info along the side. (I will provide this financial info for you to incorporate into the site design). I also need a fairly basic PHP form built to collect info from users. Info that will be emailed to a specified email address in a secure manner and in neat format. This form will have many sections, but new fields will appear based on what previous information has been provided and what further information is required. It shouldn't be too hard for an experienced programmer to do. I don't want to indulge too much else about this project until I have a winning bidder. All information will be given to the winning bidder to complete this project. Two requirements are again: 1. Good at website design (not too flashy or excessive), and 2. It doesn't have too many links out and serves a definite purpose. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform You can send me the completed files in .txt format and just inform me as to whether they need to be uploaded to the web server as php or html files.
