478809 add / modify PDF fields / add image field to one pdf form -3

this is a repost, because I am encountering several non-responsivle programmers. I will hire this evening and will be back in a couple hours. So please ONLY bid if you will respond to my messages AND WILL work to finish this by MONDAY. PDF Template 1 need to copy and paste one page and change the name of the fields. Add imageFields for each places assigned for images, don't copy past fields, if so, please change the image field names to different names. PDF Template 2 - This Template is not supporting PDFTK. this template contains many repeating fields. the name "IDcardnumber" is used for many text fields. Need to modify field names and make sure that radio button fields are not have same name. All fields should have unique names. PDF Template 3 This template is also does not support PDFTK. This also contains duplicated field names (eg, Date). please make sure to put unique field names All templates should work with PDFTK or other PDF generation tool. If using other PDF generation tool, need to install this tool on godaddy shared server.
