User Registration Form & User Data Base

Hi, I need a customer registration form which will be presented after (or before) the payment page. The form will be simple, only containing the following fields: Full Name: Username: Password: E-mail Address: Once, and only when, they register, they should then be able to gain access to the members area. I therefore need a members login page, with a password retrieval link incase they forget. The user login must work, and there cannot be duplicates on usernames. I should also be able to delete users from accessing the members area if they have requested a refund. Once the user details have been confirmed they should be redirected to the members login page. There should be no other way to access the members page without username and password. I have reasonable knowledge of html, and I have designed my website using Microsoft FrontPage. I require testing and a bug free end result. I also require future support incase of any errors. The work must be completed promptly, efficiently and must be of high quality. The look and feel can be basic but attractive. Only I, the admin should have access to delete user accounts. Please forward me your price, time frame, and exacly how you will complete the task for me. Final work will be publish on my server, via Microsoft Frontpage.
